Dictation for doctors: what happens to your audio, and why Zefmi is not a HIPAA tool

Updated 31 Oct 2026

Zefmi is not HIPAA-certified and we do not sign a BAA. Your audio goes through our relay, which stores nothing, to AssemblyAI (Deepgram is the backup), and if the live stream fails the one-shot path deletes the transcript and audio at the provider afterwards. That makes it fine for letters, admin and notes that carry no patient identifiers. Check your clinic's rules before you use it for anything more.

Everyday writing, spoken

Three made-up samples of the admin writing a doctor might dictate. None contains patient details.

What happens to your words

These are the facts that matter if you work with sensitive information. The full list of providers is on the security page.

Security and privacy in full
  • Our relay does not store your audioYour voice goes through our relay to AssemblyAI (Deepgram is the backup). The relay keeps no audio or text and logs status codes only.
  • Your history stays on your deviceYour dictation history is kept on your computer. Our servers keep word counts, not what you said.
  • We do not train AI on your wordsThe clean-up step goes through OpenRouter with data collection set to deny, so it only uses providers that do not log or train on prompts.
  • Not HIPAA-certifiedNo BAA, no medical vocabulary. Do not dictate patient identifiers unless your clinic's rules allow a tool like this, and check every word before you use it.

Coming from Dragon?

Dragon has voice commands and medical vocabularies. Zefmi has neither. Our comparison says when Dragon is still the better pick.

Questions

How dictation software works, and what Zefmi does and does not do: dictation software explained.

Is Zefmi HIPAA-compliant?

No. Zefmi is not HIPAA-certified and we do not sign business associate agreements. Check your clinic's rules before dictating patient details.

Where does my audio go?

To AssemblyAI through our relay, with Deepgram as the backup. Full list on where your voice goes.

Does Zefmi know medical terms?

It has no medical vocabulary. It types what the speech service hears, so check names of drugs and conditions before you use the text.

Do you keep my audio?

Not on our relay. The app keeps recent recordings on your own computer for 7 days by default so you can replay them; you can change that to 1 day or off.

Does it work in our clinic software?

Zefmi types at your cursor, so it works in most places you can type on Windows. Try it in your own software during the 14-day trial.

Does it work offline?

No. Zefmi needs an internet connection, because the speech recognition runs online.
